of ______________________________________________________________ give permission to our son(s) and or daughter(s) the opportunity to participate in the JJ Dushane Memorial 5 on 5 Basketball Tournament to be held at Nonotuck Park in Easthampton, MA on June 14 & 15, 2008.

By signing and participating in The JJ Dushane Memorial 5 on 5 Basketball Tournament on June 14 & June 15, 2008, the participant/s agree to release the estate and family of JJ Dushane, volunteers, Parks and Recreation Department, The City of Easthampton and their employees and all the Tournament's affiliates and sponsors of any and all damages and liability while traveling to and from and participating in The JJ Dushane Memorial 5 on 5 Basketball Tournament.

S P O R T S M A N S H I P

All players are reminded that this event is organized for charity, community spirit and camaraderie among friends and family of JJ and players from our local communities who love basketball as much as JJ did. Officials will call the game with a Zero Tolerance policy on behavior. If an official calls a technical foul, the player receiving the technical is out for the remainder of the game. The other team will be awarded two free throws - each worth one point and the ball. Any player who receives two technical fouls is disqualified for the remainder of the tournament. Any team receiving three technical fouls automatically loses the current game and is disqualified from finishing the tournament.
